Midwife Licensure Requirements

Before applying for licensure, please familiarize yourself with the general licensing policies.

 

In order to be eligible for nurse-midwife licensure, an individual must meet the following requirements:

 

Be eligible for Connecticut registered nurse licensure;

 

Foreign Trained Nurse Midwives who do not hold a current registered nurse license in another state or territory, must have successfully completed the Commission on Graduates of Foreign Nursing Schools (CGFNS) Examination and have verification of same sent directly from CGFNS to this office. 

 

Hold current certification by the American Midwifery Certification Board (AMCB); and

 

Have successfully completed thirty (30) hours of education in pharmacology for nurse-midwifery. 

 

Documentation Requirements 

 

The following documents must be submitted directly to this office from the source:

 

Individuals who have never held Connecticut registered nurse licensure must arrange for the submission an official transcript of nursing education sent directly from the nursing school;

 

Official verification of current certification by the AMCB;

 

Verification of successful completion of at least thirty (30) hours of instruction in pharmacology for nurse-midwifery.  This verification form should be completed by the Dean or Director of the nurse-midwifery educational program or, if applicable, by the Director of a post-graduate program for nurse-midwifery in which pharmacology coursework was completed.

 

Verification of all RN licenses held, current or expired.  Most states utilize the the National Council of State Boards of Nursing's Nursys System to verify licenses electronically. The purpose of the verificaton is to document license history including any disciplinary action taken.  Please select this link for a list of Nursys participating states. You may register for verifications from Nursys participating states online.

 

If you are licensed in a state that does not participate in Nursys, please send the verification form to the state and request that a written verification be sent to this office.  Most jurisdictions charge a fee for this service.  Please contact the jurisdiction for fee information.
