When an employer operates locations in multiple workers' compensation district jurisdictions, it must provide a separate Notice to Employees form for its employees who work in each different district. If you are unsure which district listing is correct for the city or town in which you work, please refer to the lists of cities and towns at the bottom of the page. Find the city or town in which you were injured and make note of the district in which that location resides. Then, refer to the employer's district listing that covers the business location where you work.
