Special information about a NEW Health Care Tax Form: IRS Form 1095-B

Overview

Enrollees in the HUSKY Health program will soon be receiving a new tax form in the mail.  This is the ‘IRS Form 1095-B,’ required by federal law to help you prepare your federal tax return.
 
In January 2016, the Department of Social Services will be sending IRS Form 1095-B to all Medicaid and Children’s Health Insurance Program (CHIP) members who were enrolled for any portion of 2015.
  • You will receive your Form 1095-B in the mail by January 31, 2016.

  • Form 1095-B is an official tax document which serves as proof of enrollment in ‘minimal essential health coverage.’  All HUSKY Health programs which provide full medical coverage qualify for this.

  • Use the enrollment information provided on Form 1095-B to complete your annual federal tax return. 
This is important because the Affordable Care Act’s ‘individual responsibility mandate’ requires most Americans and qualified non-citizens to obtain and maintain minimum essential health coverage each month or obtain an exemption – otherwise, a tax penalty may be charged.
