Order Supplies
Birthing Centers, Midwives, and Primary Care Physicians can order Newborn Screening bloodspot specimen cards by contacting the Outfitting Room at the Department of Public Health Laboratory via phone, fax, or email. Each card will come with an envelope as well as instructions for collection and an informational hand out for parents. Please be sure to let our Outfitting staff know how many cards you require.
