Department of Public Health Survey

The Department of Public Health (DPH), Title V Children and Youth with Special Health Care Needs (CYSHCN) Program is conducting a statewide needs assessment that requires input from youth with special health care needs (YSHCN) age 12+ and parents/caregivers of YSHCN on Core Transition Outcome Data. There are two versions of this survey: a youth/young adult version and a parent/caregiver version. Both surveys are available in English and Spanish.
The purpose of the surveys is to assess transition experiences of youth with special health care needs in Connecticut.  These survey questions assess key transition issues including: health insurance; doctors and other healthcare providers; resources; health care; school; work; housing; transportation and other transitions.
We recognize that is this a busy time for YSHCN and their families/caregivers (i.e. summer vacation, back to school time, etc.). However, Youth/Family input is VERY important to us. Results from the surveys will be used to guide DPH’s Transition Initiative and evaluate the State Implementation Grants for Integrated Community Systems for CYSHCN Grant (aka DPH Transition Grant). We ask that YSHCN and their families/caregivers complete the surveys as soon as possible but no later than Friday September 9th, 2011. The surveys will take approximately 15 minutes to complete.

Then choose from the English or Spanish Youth/Young Adult or Parent/Caregiver versions to complete. The surveys are also available in hard copy (see attached). You may mail the hard copies of the survey to address below or fax them to 860-509-7720.
