Optometry Licensing Requirements

Before applying for licensure, please familiarize yourself with the general licensing policies.
 
An applicant for licensure shall meet the following requirements:
 
Have graduated from a school or college of optometry accredited by the Council on Optometric Education of the American Optometric Association; and
 
Has successfully completed Part I Basic Science, Part II Clinical Science and all three components of the Part III Patient Care and the Treatment and Management of Ocular Disease (TMOD) Examinations of the National Board of Examiners in Optometry (NBEO).
 
Documentation Requirements
 
An applicant for licensure shall arrange for the submission of the following documentation directly to this office:
 
An official transcript from the accredited school or college of optometry indicating degree earned and date granted;
 
Official verification of Parts I, II, III and TMOD of the NBEO Examinations directly from the NBEO;
 
If applicable, official verification, sent directly from each state licensing authority where the applicant holds or has held a license.  Please contact the jurisdiction prior to submitting the form as some require a fee for this service;
 
A completed application with photograph attached.  Applications are only accepted online.  Please select this link to apply online.  The application fee is $565.
